Saturday, October 3, 2020, 9:30am-11:30pm (rain or shine)
Starts at Historic Axe Factory Building, Downtown Collinsville, CT
Parking Available in gravel lot OFF OF MAIN ST.

Conduct a COVID-19 Self-Assessment BEFORE your arrival
In the interest of ensuring a safe and healthy practice environment, we ask that you carefully review this self-assessment check prior to your arrival at THE WALK SITE. At all times health protocols and requirements need to be followed.

SELF-ASSESSMENT

HAVE YOU OR A MEMBER OF YOUR HOUSE HOLD TRAVELED ABROAD OR DOMESTICALLY?  
All participants to THE WALK who have traveled to U.S. locations; Alabama, Arkansas, Arizona, California, Florida, Georgia, Hawaii, Iowa, Idaho, Illinois, Indiana, Kansas, Kentucky, Louisiana, Maryland, Minnesota, Missouri, Mississippi, Montana, North Carolina, North Dakota, Nebraska, Nevada, Oklahoma, Puerto Rico, South Carolina, South Dakota, Tennessee, Texas, Utah, Virgin Islands, Virginia, Washington (Metrics for Washington are currently under review, status may change), Wisconsin, OR internationally or have traveled on a cruise in the last 14 days are subject to self quarantine as per CT state protocol. Note: this does not include travel as part of the commute to/from work.

HAVE YOU OR SOMEONE YOU HAVE BEEN IN CLOSE CONTACT WITH BEEN DIAGNOSED WITH COVID-19?
Persons with close contact* with a person who has been diagnosed with 2019-nCoV Acute Respiratory Disease (COVID-19) by testing or Doctor diagnosed. *Close contacts are people who are within about 2m (6 feet) for about 30 minutes or more.

IF YOU ANSWER YES TO QUESTIONS ABOVE:
We ask you to PLEASE DO NOT PARTICIPATE IN THE WALK EVENT, but self-quarantine in accordance with the best practices requirement for 14 days starting the day you or your household member arrived back in the State of Connecticut.

DAY-OF ASSESSMENT:
Do you have:
1. Fever (greater than 38°C/100.4°F) or
2. Signs and symptoms of respiratory or flu-like illness

If either of the above symptoms are present, PLEASE DO NOT ATTEND THE WALK EVENT. You may support the walk by making an online donation.


UPON ARRIVAL TO THE WALK EVENT:
A. Please wear a mask to the WALK EVENT.
B. Please avoid touching (e.g. hugging, shaking hands, etc.) of course a wave, or palms together with a half bow is appropriate.
C. When greeting each other maintain a safe distance of at least 6-feet.
D. During THE WALK Please make sure to maintain a safe distance between
you and the walker in front of you.
